Overall, this is a well-made cool pack and neoprene sleeve with straps that secure well. I’m 5’4” and got the Large. The cold penetrates deeply. The main issue I have is that gravity pushes down the gel that covers the top of your shoulder toward the ends of the neoprene case that hang over your chest and shoulder blades. The result is that the gel layer is “thinner” on top. Having more “seams” in the gel packs would maybe fix this. The other thing is that the enclosed instructions show to wrap the extender straps from the front to the back. Well, if you have a shoulder injury it can be difficult to reach behind to fasten it unless you have a helper. I reversed the instructions and fastened it from back to front.
